House Check Form

Going on vacation? Use this form to request a periodical house check by the City of Lookout Mountain Police.

Once you submit the online form, you will be contacted to confirm the house check. Without your confirmation, the house check will not take place.

You may also submit a signed physical copy to the police department (see file link below).
